Volunteer Permission Slip (for High School Students) has two options:
- A parent/guardian can complete and sign the form electronically here. This is the preferred method for submitting this form.
- A form can be printed and completed here.

Adult volunteer note: background screenings are required. At this late stage, we can only accept volunteers that are already in the Family Care Safety Registry. (Don’t worry! Many people have been added to the registry through scouts, school districts, and other volunteer activities involving kids.) You can check to see if you are in the registry here.
